ABSTRACT

Background The prevalence of osteoporosis and osteopenia is higher among underground coal miners than surface workers. The special underground work environment and unhealthy habits such as smoking, drinking, and a high-salt diet may lead to changes in bone metabolism, increasing the risk of fragility fractures and placing a heavy economic burden on individuals and society. Objective To identify potential factors influencing fragility fractures among coal miners in different working environments and to provide a basis for targeted preventive measures to reduce the occurrence of fragility fractures. Methods Male participants who attended at least one of the physical examinations in Kailuan Group between June 2006 and December 2020 were included in the study. The participants were divided into two groups based on their working environment: surface or underground. A case-control study was conducted, where patients with new fragility fractures served as the case group and participants without fragility fractures served as the control group. The two groups were matched with a case:control ratio of 1:4 by age (±1 year) and the same year of physical examination. The matching process was repeated twice, once for the surface working population and once for the underground working population. The analysis of risk factors was conducted using conditional logistic regression models. Results Among a total of 113138 employees in Kailuan Group, 82631 surface workers and 30507 underground workers were included, respectively. The number of individuals who suffered fragility fractures was 1375, accounting for 1.22% of the total population. The incidence of fragility fractures in underground workers was significantly higher than that in surface workers (1.63%>1.07%, P<0.001). The results of conditional logistic regression model showed that current smoking (OR=1.26, 95%CI: 1.05, 1.51), manual labor (OR=1.37, 95%CI: 1.06, 1.78), diabetes (OR=1.26, 95%CI: 1.04, 1.54), sinus tachycardia (OR=1.81, 95%CI: 1.23, 2.66), history of stroke (OR=1.51, 95%CI: 1.09, 2.09), education at college and above (OR=0.65, 95%CI: 0.45, 0.95), high income level (OR=0.69, 95%CI: 0.54, 0.90), elevated hemoglobin (OR=0.91, 95%CI: 0.85, 0.98), and elevated total cholesterol (OR=0.90, 95%CI: 0.82, 0.99) were associated with fragility fractures in the surface working population of coal mines; current smoking (OR=1.48, 95%CI: 1.17, 1.87), current drinking (OR=1.26, 95%CI: 1.01, 1.56), manual labor (OR=2.64, 95%CI: 1.41, 4.94), history of dust exposure (OR=1.28, 95%CI: 1.03, 1.58), and obesity (OR=0.72, 95%CI: 0.52, 0.96) were associated with fragility fractures in the underground working population of coal mines. Conclusion In preventing fragility fractures, special attention should be paid to the bone health of underground workers engaged in manual labor or having a history of dust exposure. It is important to correct their unhealthy behaviors in a timely manner, such as smoking and drinking, and to appropriately increase body weight to prevent fragility fractures. For surface workers, particular attention should be given to the high-risk group for fragility fractures, such as low family income per capita, manual labor, and having a history of stroke or diabetes; in addition, close monitoring of their resting heart rate, hemoglobin levels, and total cholesterol levels may help prevent fragility fractures.

ABSTRACT

Objective:To explore the relative importance of palliative care knowledge, attitude towards palliative care and healthy work environment of emergency care nurses in predicting perceived self-competence in palliative care based on dominance analysis, and to provide empirical evidence for formulating the optimal training course to improve perceived self-competence in palliative care. Methods:Totally 415 emergency care nurses from 22 tertiary hospitals in Hubei province were conducted a cross-sectional survey to complete the general information questionnaire, the Palliative Care Nursing Self-Competence Scale, the Hospice Knowledge Quiz for Nurses, the Frommelt Attitudes Toward Care of the Dying Scale Form B, and the American Association of Critical Care Nurses Healthy Work Environment Assessment Tool from January to March 2021 using convenient sampling method. Dominance analysis was used for statistical analysis. Results:The total score of perceived self-competence in palliative care of 415 emergency care nurses was (225.48 ± 60.61) points, the total score of palliative care knowledge was (12.44 ± 3.83) points, the total score of palliative care attitude was (97.35 ± 9.07) points and the total score of healthy work environment was (72.00 ± 11.99) points. Perceived self-competence in palliative care was positively associated with palliative care knowledge ( r=0.181, P < 0.01), attitude towards palliative care ( r=0.232, P<0.01) and healthy work environment ( r=0.393, P<0.01). Dominance analysis revealed that the healthy work environment, palliative care attitude and palliative care knowledge accounted for 74.15%, 14.97% and 10.88% of the variance of perceived self-competence in palliative care, respectively. Conclusions:The largest degree of prediction for emergency care nurses′ perceived self-competence in palliative care is healthy work environment, attitude next and the third is knowledge. It suggests that promoting a healthy work environment is an important way to improve nurses′ perceived self-competence in palliative care, and improving knowledge level and cultivating positive attitudes also will be better.

ABSTRACT

Abstract BACKGROUND: Brazil's Family Health Strategy is based on a primary healthcare model, which is considered to have case resolution capacity, with physicians at its center. OBJECTIVES: To evaluate the levels of occupational stress and work engagement among primary healthcare physicians. DESIGN AND SETTING: Cross-sectional study conducted in 2017, in São José do Rio Preto, São Paulo, Brazil. METHODS: A non-probability sample including 32 physicians from family health teams was used. Three self-applied instruments were used: a scale developed by the researchers seeking sociodemographic and professional variables, the Work Stress Scale and the Utrecht Work Engagement Scale. RESULTS: Female professionals (59.4%), permanent employees (56.3%), workload of 40 hours per week (59.4%) and 3-10 years of acting in primary care (68.8%) were more prevalent. Six professionals (19.4%) exhibited significant stress (score ≥ 2.5). The main stressors were lack of prospects for career growth (2.9 ± 1.3), form of task distribution (2.7 ± 1.0), poor training (2.7 ± 1.2) and insufficient time to perform the job (2.6 ± 1.2). Levels of work engagement ranged from 4.3 to 4.6 and were rated as high in all dimensions. Physicians with occupational stress had average levels of work engagement, whereas those without occupational stress had high levels of work commitment. CONCLUSIONS: A notable percentage of the physicians were experiencing occupational stress. The physicians had high levels of work engagement. Occupational stress was negatively correlated with work engagement, and it significantly compromised physicians' levels of work engagement and interfered with their positive relationship with the work environment.

ABSTRACT

Healthy work environments that maximise the health and well-being of nurses are essential in achieving good patient and societal outcomes, as well as optimal organisational performance. While studies have been conducted on healthy work environments, there is no available evidence that an integrative literature review summarising best-practice recommendations related to healthy work environments has been conducted before. This review aimed to summarise existing best-practice recommendations related to a healthy work environment for nurses. An integrative literature review following the approach adapted from Whittemore and Knafl was used. Existing guidelines related to healthy work environments for nurses were searched. EBSCOhost (CINAHL, Medline), Biomed Central, Science Direct, PubMed and Google Scholar and organisational websites via Google were searched, followed by a citation search. Twelve guidelines were identified for data extraction and synthesis, and themes were subsequently formulated. Four themes emerged from the integrative literature review regarding a healthy work environment for nurses: (1) the need for effective nursing leadership, (2) effective communication as central to enhancement of a healthy environment, (3) effective teamwork as an integral part of a healthy work environment and (4) the need for professional autonomy. In summary, a healthy work environment for nurses requires leadership, effective communication, teamwork and professional autonomy.

This study aimed to understand the configuration of the work environment of nursing professionals in the hospital scenario, considering the context of the COVID-19 pandemic. This is a single case study with a qualitative approach carried out in a large public hospital in Belo Horizonte/Minas Gerais. The sample of this study was composed of nurses and nursing technicians who work in the institution, from the care and administrative areas, with more than two years of admission, totaling 32 professionals. The data were collected between December 2021 and March 2022. After treatment and analysis, three categories emerged: Healthy Work Environment: conception of workers; Pathways for the achievement of a healthier work environment: potentialities and challenges; The pandemic: impacts on the life and work environment of nursing professionals. The need for investments in the promotion of a healthy work environment in the physical, psychosocial and personal health dimensions was evident, and it is necessary that these values be better incorporated into the institution's culture. We identified in the statements of the workers the feeling of welcome and satisfaction with the workplace, as well as organizational factors and practices that contribute to a healthier work environment, such as the institution's closeness to the community, good interpersonal relationships among workers, and benefits offered by the institution. However, the findings of this study point to the need for attention to some factors such as the physical structure, institutional communication, high demand for work, and an imbalance between the volume of activities and the response capacity of the professionals. We also identify the need for investments in professional valuation, incentives for improvement and the insertion of nursing workers in decision-making bodies. When evaluating the changes caused by the pandemic in the work environment and professional practices, it was evidenced challenges such as changes in the physical structure, constant changes in routine, considerable increase in the high demand of work, fear of ontamination, absenteeism due to contamination by COVID-19, besides the difficulty in dealing with the instability and death of users. These issues decreased the quality of the work environment and brought physical and emotional impacts such as insecurity, stress, and anxiety. In this period, the institution adopted measures to ensure the protection of workers and users and to try to minimize the effects of the pandemic, such as training, increased staff, reinforcement of worker safety practices, changes in institutional flows, and investments in the promotion of well-being. We conclude that the institution has strategies and activities related to the theme of this study, which were adopted before the pandemic period, but also during it. However, sometimes these measures are lost because they are not well articulated and have a temporary character. Thus, for them to be effective, sustainable and have a greater reach, it is necessary to have a broader look at the needs presented by workers, stimulate qualified listening, greater insertion of professionals in decision-making bodies and robust planning that supports the actions implemented in the long term, bringing positive impacts.

ABSTRACT

ABSTRACT: Studies have been developed in work contexts, which emphasize the importance of privileging diversity and developing practices of inclusion, with a view to equal rights and the welcoming of multiple ethnicities, genders and people with disabilities. In this sense, this study aimed to analyze the Brazilian scientific production about diversity and inclusion of people with visual impairment in work contexts. It consists of a systematic review of scientific articles, published until 2020, in two electronic databases. After a systematic search, three articles were identified, described and analyzed using the categories "potentialities" and "difficulties of the inclusion process". Te identified scientific production, perhaps because it is in its initial stage, reaffirms the importance of education, public policies, legislation and insertion or reintegration into work environments, which can be considered stages, albeit fundamental, prior to the inclusion process. In addition, the difficulties faced by people with visual impairments at work are emphasized, not paying more attention to the potentialities of this insertion process for people, as well as for organizations and society. Te importance of educational proposals aimed at promoting inclusion is highlighted, which involve, in addition to spaces formally recognized as educational, the work contexts. Terefore, it is understood that studies as this one can foster the expansion of knowledge production, as well as discussions to create culturally sensitive inclusion policies and practices for people with visual impairments in the world of work.

ABSTRACT

Abstract Introduction Mental health in training physicians is a growing issue. The aim of this study was to investigate emotional distress in psychiatry residents. Method This web-based survey evaluated 115 (62%) psychiatry residents in training in the Brazilian State of Rio Grande do Sul. The DSM-5 Self-Rated Level 1 Cross-Cutting Symptom Measure-Adult, the Patient Health Questionnaire-2, the Alcohol Use Disorders Identification Test-concise, and the Maslach Burnout Inventory were all administered. Linear regression models were estimated with burnout dimensions as dependent variables. Result Positive screening rates were 53% for anxiety, 35.7% for somatization, 16.5% for depression, and 7% for suicidal ideation. Half of the male residents were at risk of alcohol abuse and dependence. Regarding burnout, 60% met criteria for emotional exhaustion, 54.8% for depersonalization, and 33% for low personal accomplishment. The most consistent risk factors were the nature of the relationships with preceptors, relations to the institutions themselves, age, and the quality of relationships with family. Conclusion Besides disconcerting rates of psychiatric symptoms, the study revealed that characteristics of the workplace (i.e., the nature of relationships with preceptors and relations to the institution) can be regarded as potential targets for development of interventions aimed at improving mental health during training periods.

ABSTRACT

ABSTRACT One of the topics that have not been studied in depth is the evaluation of the organizational climate in companies of the agricultural sector under multivariate statistical analysis. Thus, the purpose of this study is to construct and validate an instrument to measure the organizational climate of technified dairy cattle raising companies, under a mixed approach, with an exploratory scope of psychometric technological type, developing an analysis of categorical variables. The study sample consisted of 45 farms, which were analyzed under 9 dimensions supported by 97 items under the Likert scale, where the Cronbach's alpha obtained was 0.9044395, which shows that the data are reliable and that there is internal consistency among the questions of the questionnaire. The analysis of the results was carried out using multivariate techniques of simple correspondence for each of the established dimensions, allowing the analysis of the problem from a psychosocial approach. The dimension of greatest concern was the social context. This showed problems related to the scarce opportunity for social interaction, few spaces for recreation, and family recreation; the best-evaluated dimension was that of Leadership and participation, showing a very good relationship with the boss. In general, it can be said that the highest percentage of the grouping of responses is around option always, that correspond to the ideal qualification for each item, represented in 57.32%, which indicates a behavior close to the ideal in certain variables analyzed.

ABSTRACT

Objective: To study the improvement of the mental health of Japanese male registered nurses, we investigated the predictors associated with the Japanese version of the 12-item General Health Questionnaire (GHQ-12). For those predictors, we focused on environments with many female registered nurses and female patients’ refusal to accept nursing services from male registered nurses.Materials and Methods: This study was conducted in 93 hospitals in the Aichi, Gifu, and Mie prefectures of Japan. The analyzed subjects were 1,216 full-time male registered nurses. The average age (standard deviation) was 34.5 (9.2) years. Multiple logistic regression analysis was conducted. A GHQ-12 score of ≤3 indicated good mental health, and a score of ≥4 indicated poor mental health. “Mental health” was a dependent variable. The independent variables were: “Age”, “Male registered nurses as important partners on the job”, “Female registered nurses’ good manners toward male registered nurses”, “Fairness regarding male registered nurses’ promotions”, and “Female patients’ refusal to accept nursing services from male registered nurses”.Results: There were 728 (59.9%) full-time male registered nurses with good mental health and 488 (40.1%) with poor mental health. The mental health of the male registered nurses who had negative feelings regarding, “Male registered nurses as important partners on the job” was significantly worse, and that of those who had negative feelings regarding, “Female registered nurses’ good manners toward male registered nurses” was also significantly worse. Conclusion: Female registered nurses must recognize that their attitudes toward male registered nurses influence the mental health of male registered nurses. Hospital managers should provide male registered nurses who cannot establish appropriate relations with female registered nurses with consultation opportunities. Such organizational action by managers is necessary.

ABSTRACT

@#One of the greatest healthcare challenges for today's generation is the improvement of the nursing work environment. Despite the challenges brought by the unhealthy working environment and high workload demand, Filipino nurses stood up high in maintaining professionalism towards their work. The goal of this study is to determine the relationship between the work environment and the workload of staff nurses in level 2 hospitals in Cavite, Philippines. Quantitative non-experimental descriptive correlational design was used, and the standardized questionnaires namely: Practice Environment Scale of the Nursing Work Index (PES-NWI) and NASA Task Load Index (NASA-TLX) were used to gather data from 70 respondents. Results showed that majority were female, belonging to Millennial age (23-38 years old), had baccalaureate degree in Nursing, with a gross monthly salary of Php 15,000.00- 19,999.00, being in the institution for more than 6 months to 5 years, and have an average nurse to patient ratio of 11.30. Their level of work environment was favorable in all subscales with a composite score of 3.13 (SD= 0.570). Their perceived workload was high, with a mean score of 71.24 (SD= 12.78). Likewise, nurse to patient ratio had a significant relationship with nurse manager ability, leadership, and support of nurses (X2= 94.26, p= <.001) and staffing and resource adequacy (X2= 75.06, p= 0.002). Moreover, significant relationships among the variables of age and physical demand (p= .004), length of institutional experience and physical demand (p <0.001), educational attainment and performance (p= .018), educational attainment and effort (p= .005), and length of institutional experience and effort (p= .001). Last, there is no statistically significant correlation between the overall work environment and the overall workload of nurses (r= -.084, p= .491). These findings underscore the importance of measures aimed at enhancing the work environment and workload of staff nurses to prevent further challenges among them.
